    Chitosan and Its Modifications: Are They Possible Vehicles for Gene Therapy?

    [Extract] In gene therapy, the most important step is how to effectively deliver the therapeutic gene to the target cells or organ. At present, there are two methods, which are those using a viral and a non-viral vector system. The most common viral vectors that have been used include retroviruses, herpes simplex viruses, lentiviruses, adenoviruses and adeno-associated viruses (Oligino et al., 2000). The advantages of most viral vectors are high transfection efficiency and fast transcription of the foreign material inserted in the viral genome. However, a number of disadvantages have also been described, such as poor target-specificity, low capacity to incorporate foreign deoxyribonucleic acid (DNA) sequence to their genome (Mansouri et al., 2004), toxic and inflammatory effects, wild– type mutations, potential oncogenic effects (Lee et al., 1998), and, in particular, unwanted immune responses. In clinical trials of gene replacement therapy using viral vectors, significant adverse effects have been reported, including a fatal inflammatory response associated with adenoviral vector (Raper et al., 2003), and the development of acute leukaemia in recipients of ex-vivo, adenoviral vector-transduced hematopoietic cells (Woods et al., 2006). Intravenous adenoviral vector has also resulted in high liver toxicity due to uptake by hepatocytes or Kupffer cells of the liver reticular endothelial system, immediately following systemic administration

    The historical decline of tobacco smoking among Australian physicians: 1964–1997

    <p>Abstract</p> <p>Background</p> <p>Physicians occupy an important position as tobacco control exemplars and their own smoking habits are known to influence how effective they may be in such a role.</p> <p>Methods</p> <p>A comprehensive review of all published manuscripts describing tobacco usage rates and tobacco control activities in the Australian medical profession between 1964 and 1997.</p> <p>Results</p> <p>Some of the earliest surveys revealed that around one-quarter of Australian physicians were smoking in the mid twentieth century, a rate which rapidly declined in the 1970s and 1980s, with reductions beyond that achieved by the general population.</p> <p>Conclusion</p> <p>Overall, our review suggests that not only do contemporary Australian physicians smoke at very low rates when compared internationally, but that an active professional community can also make a real difference to the lifestyle choices of its own members.</p

    Acute Lymphatic Filariasis Infection in United States armed forces personnel deployed to the Pacific area of operations during World War II provides important lessons for today

    The deployment of United States (US) Armed Forces personnel into the central Pacific islands of Samoa and Tonga, which is highly-endemic for lymphatic filariasis (LF), resulted in thousands of cases of the acute form of this disease and greatly reduced their ability to carry out their mission. The major driving factor for the intensity of transmission was the aggressiveness and efficiency of the Aedes species mosquito vectors, especially the day-biting Ae. Polynesiensis. The paper reminds us of the danger that tropical diseases can pose for troops sent into endemic areas and constant and careful surveillance that is required to prevent rapid resurgence of Aedes-transmitted LF in populations, where the LF elimination program has been successful

    This Special Issue focuses on recent global research on the current coronavirus (COVID-19) pandemic. The disease is caused by a novel virus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The International Committee on Taxonomy of Viruses (ICTV) named the virus SARS-CoV-2, as it is genetically related to the coronavirus responsible for the SARS outbreak of 2003. While related, the two viruses are quite different in their behaviour. At the time of submission for publication (7 January 2022), COVID-19, named by the World Health Organization (WHO) on 11 February 2020, had caused more than 296.5 million cases and over 5.5 million deaths with over 2.6 million new cases in the past 24 h. The COVID-19 pandemic has greatly affected the capacity of health systems providing essential health care, but more than 9.195 billion vaccine doses have been administered as of 10 January 2021. There have been 22 papers published upon peer review acceptance in this Special Issue, including one editorial, twelve research papers, three review papers and seven other papers, including one perspective, two case reports, one brief report, two viewpoints and one commentary. They each contribute to a much better understanding of COVID-19

    Exposure rate of needlestick and sharps injuries among Australian veterinarians

    Background: Needlestick and sharps injuries (NSI) represent an important occupational health issue in veterinary practice. Little is known about the distribution and correlates of NSI among Australian veterinarians. Methods: A questionnaire-based NSI survey was mailed to 1094 veterinarians registered with the Veterinary Surgeons Board of Queensland during 2006. Results: A total of 664 surveys were returned from 1038 eligible participants (response rate 64.0%) with 56.8% being male, around one-third in the >50 years age group and about half aged 3150 years. Just over two-fifths were working in small animal practice only. Around three quarters (75.3%) reported suffering at least one NSI in the previous 12 months, while 58.9% reported suffering from at least one contaminated NSI during the previous 12 months, which crudely extrapolates to an exposure rate of 75.3 and 58.9 NSI per 100 person-years respectively. Risk factors for contaminated NSI were female gender, working in small or mixed animal practice, being less experienced, seeing more patients per week and working longer hours per week. The most common causative devices were syringes (63.7%), suture needles (50.6%) and scalpel blades (34.8%). Conclusion: The exposure rate of NSI is high for Queensland veterinarians and clearly remains a major occupational health problem. Current guidelines and strategies to reduce NSI in veterinary practice should be promoted, but appear to be adapted from human health care. Studies to understand why veterinarians have such high NSI rates are required to not only identify risk factors for NSI, but also to determine attitudes and beliefs about NSI. From these studies specific strategies for veterinarians can be designed and trialed to develop evidence-based guidelines and policies that are effective in decreasing the exposure rate of NSI in veterinary practice

    Developing drowning prevention strategies for rivers through the use of a modified Delphi process

    Introduction Internationally, rivers are a leading drowning location, yet little evidence exists evaluating river drowning prevention strategies. This study aims to use expert opinion to identify strategies more likely to be effective. Methods Using a modified Delphi process, a virtual panel of 30 experts from 12 countries considered, grouped and prioritised strategies for river drowning prevention. Proposed strategies were assessed against known evidence and suitability in high-income countries (HICs) as well as low-income and middle-income countries (LMICs) using expert opinion. The final phase consolidated a list of strategies whose effectiveness was assessed against 10 evidence-based river drowning scenarios. Results An initial list of 424 prevention strategies was refined to 22. After being assessed against the 10 scenarios, a final list of 13 strategies was derived. Strategies addressed alcohol consumption around rivers, flood mitigation, improving child supervision, learning to swim, increased lifejacket wear and achieving community-wide resuscitation skills. Discussion While all 13 strategies were assessed as being effective in both LMICs and HICs by at least 60% of the respondents, further work is required to define river drowning at a country level and therefore allow for effective solutions to be developed, particularly in LMICs. No strategy will be effective in isolation and must be implemented alongside policy and behaviour change, public awareness and education. Evaluation should be incorporated as part of any future implementation of strategies. Conclusion This Delphi process identified 13 drowning prevention strategies for rivers. Further research is required to validate the efficacy of these findings through implementation and evaluation

    Neglected and Emerging Tropical Diseases in South and Southeast Asia and Northern Australia

    This Special Issue focuses on recent research on the important emerging and neglected tropical diseases (NTDs) in South and South East Asia and Northern Australia. This region stretches from Pakistan in the west to the Philippines in the east, and includes Afghanistan and countries to the east, the Indian subcontinent, mainland South-East Asia, and the tropical regions of Australia. Many of these areas are highly endemic for important NTDs and other tropical diseases, including lymphatic filariasis (LF), soil-transmitted helminthiases (STH) such as hookworm infection, trichuriasis, ascariasis and strongy loidiasis, rickettsial diseases and arboviral diseases. Several of these diseases are targeted for elimination or enhanced control by the World Health Organization (WHO) in the next 5 to 10 years, although some have chronic lasting sequelae and disability needing lifelong management. Control methods used include preventive chemotherapy, enhanced screening and treatment, intensified disease management, vector control, interruption of human to animal transmission, environmental/sanitation improvements and disability prevention/mitigation
